  • We offer hospice assistance programs as well as support care during life transition, of for recovery after trauma, medical surgery or at the end of life. We can provide this kind of service at-home, in a hospital or a nursing home.

Program Overview

Diamond Care, Inc. offers a unique, high-quality hospice assistance program that provides compassionate care for people facing a life-limiting illness or injury. Our Hospice Assistance features a team-oriented approach that supports expert medical care and pain management, to help our clients continue to perform their Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL). We also work with spiritual advisers and members of the clergy to ensure that the spiritual needs of our clients and their loved ones are met.

The mission of DCI Hospice Assistance is to provide palliative care that ensures that each of our clients exercises their right to die with dignity and that their families will receive necessary support during a difficult time.

Who Needs Our Services?

  • People who have suffered major trauma due to an accident or surgery.
  • People who are diagnosed with dementia due to Alzheimers, Parkinson’s Disease, Lewy Bodies or other diseases or syndromes.
  • People suffering from debilitating cancers.
  • Autistic adults.
  • Adults with other illnesses, disorders or disabilities that cause them to lose their independence.
  • People on hospice or who are receiving palliative care.
  • Adults who have disabilities and want more independence and enjoyment from life.
